Dr. Rome Talks Hair Loss Treatments With Strut Health

“My name is Dr. Rome. I’m the founder/CEO of Barber Surgeons Guild. We’re a medical practice, we also feature a grooming salon. We focus on hair restoration and treatments for hair care.”
“There is no cure at this time for hair loss. We have really good treatments, thankfully, to greatly slow down if not almost stop hair loss depending on the genetic predisposition to the amount of hair loss someone is going to have, which is variable.”
“Most people come in with male pattern hair loss or female pattern hair loss, which is called androgenetic hair loss, which is hormonally mediated hair loss that occurs with genetics, family history. It’s also thankfully the most easily treatable type of hair loss.”
“We have medications that are oral, things like Finasteride, Dutasteride, are oral medications that are quite effective. And then we have topical medications which are preparations typically of these same compounds mixed with some other things, including Minoxidil.”
“Finasteride is a molecule that blocks the breakdown of testosterone into one of its active metabolites called DHT. DHT is what causes male pattern thinning. So, the area of hair here that you see that gets lost [gesturing to the crown and frontal portion of the scalp] with male pattern thinning is called DHT sensitive hair. And, vs, the DHT resistant hair here that never thins out [gesturing to the back of the scalp]. So, by lowering the DHT levels, which again is what Finasteride is doing, we’re protecting this DHT sensitive area and that’s how it slows down hair loss.”
“There is a difference between topical and oral Finasteride. My personal feeling is if someone is willing to take a medication I will prescribe that first. Really, because we have the most evidence and it’s been around the longest. That said, there’s a great deal of people that just don’t want to take a medicine for one reason or another. Or, that they have been one of the unlucky people that have had side effects from oral medication, in which case the topical is a great option. I personally have never seen anyone that’s had any side effects from topical use of Finasteride.”
“The truth of the matter is, in the literature, they say 1-2% of patients will have some side effects. It’s probably, in my opinion, an overestimation. The side effect that you can have from Finasteride, when you take it orally, specifically, for some people is that it can for some reason affect their mood. I don’t believe there is any such permanent side effect from taking it. Anyone that I personally prescribed to, which is thousands of people, anyone that has had side effects to any degree, within a day or two of stopping taking it, it has gone away.”
“Dutasteride is in the same family as Finasteride, it also blocks the same enzyme, it actually blocks it a little more effectively than it does for Finasteride. Therefore, it lowers your DHT levels a little bit more than Finasteride does. It was a medicine that continues to be used for a different indication, similar to Finasteride at a larger dose. That said, we, as doctors, can use it off-label with great success to slow down hair loss as well.”

“Dutasteride Vs. Finasteride. The Dutasteride will lower the DHT levels a little bit more, and it is a little bit more effective than Finasteride. People ask sometimes about side effect profile of one vs. the other. I’ve seen that people have a side effect with one and then they try the other and then they don’t. You know, it’s worth a try if you’ve had an issue with one, try the other one.”
“You know, often people say “oh, I don’t want to take a medicine. I’d just rather do something all-natural.” And then I tell them, if you want to do something all-natural, you can lose your hair and that’s an all-natural thing and there’s nothing wrong with that, if that’s what you want. But, if you want to defy your genetics, which is to lose your hair, you know, we have really good medications that have low side effect risks.”

“If you are experiencing hair loss, my recommendation is to have a conversation about it, figure out what your options are, and consider some form of treatment to stop it from getting worse. That way you can keep your options open, and if you decide that it’s not important for you to maintain your hair, fine. But, if you’re not ready or sure about how you want to proceed, my recommendation is to maintain your hair as best you can. We have great options available and it’s really worth looking into some of those.”
“We have patients that come to us all the time that want to learn about hair restoration, and unfortunately, they just are not candidates because they have lost more hair and we can’t treat them at that point, unfortunately, there’s just not enough hair that we can move. So, we really stress early intervention with medicines.”
